2. Baptismal Certificate – original & photocopy (for Catholics)
3. Original Form 138 (Report Card)
4. Certificate of Good Moral Character/Honorable Dismissal
5. 4 pcs. 1.5 x 1.5 colored pictures w/ white background
6. Long Brown Envelope
7. Entrance Test Fee – New Students 200.00 – Temporarily Suspended
   – Returnees 100.00 – Temporarily Suspended

Old Students:

1. Original Form 138 (Report Card)
2. Have not been subjected to any previous case(s) of violation of any of the school rules and regulations and have settled all previous financial obligations.
